| Dr. Maleki |

Dr. Maleki began his La Habra Practice in 1984 when he joined Friendly Hills Medical Group, a large multi-specialty medical group in the area. He stayed with the group until 2000 and served also as the Chairman of the Family Practice Department for Friendly Hills Group and later he became the Regional Chairman of the Family Practice Department of MedPartners Medical Group, a larger group to which Friendly Hills Medical group was affiliated. He also chaired several important committees such as the Medical Practice Guidelines Committee for several years. After 2000, Dr. Maleki started his own practice in partnership with three other Primary Care Physicians in La Mirada, California and continues to serve this community to this day.
Dr. Maleki’s undergraduate and medical school training were all completed in London, England before he and his family moved to California in 1982. A member of the American Academy of Family Practitioners for almost 20 years, Dr. Maleki was awarded Fellowship of the Academy several years ago. He has also been the Chairman of Imperial Primary Care Medical Group since its inception in 2000. His interests include running, laser cosmetic procedures, and spending time with his wife and children. |

| Dr Rubalcava |

Born and raised in the beautiful San Gabriel Valley of California, Dr. Rubalcava graduated from the Revelle College at the University of California at San Diego (UCSD) with a Bachelor Degree in Biology. He then proceeded to obtain his Medical Degree from the University of Illinois at Chicago. His medical training was then finalized back in California, at the Family Practice Residency Program at USC-Presbyterian Intercommunity Hospital in Whittier.
Dr. Rubalcava is currently Board Certified with the American Board of Family Practice and a member of The American Academy of Family Practice. He is also a partner with Imperial Primary Care Medical Group in the city of La Mirada. Fluent in both English and Spanish, Dr. Rubalcava enjoys listening to music and spending time with his wife and daughter.

I have been in practice in La Habra and La Mirada area for over 20 years. During this time I have served as Chairman of the Department of Medicine for both Friendly Hills Medical Group and Whittier Hospital. I have also served as Vice Chief of Staff for the former La Habra Hospital and am currently Vice-President of the La Habra Heights County Water District.
I graduated Magnum Cum Laude from Thiel College and completed my Medical Degree at the University of Maryland. Following the completion of my studies, I moved to California in 1975 where I completed my residency in Internal Medicine with an added qualification in Geriatrics.
Currently, my primary hobby is school. I returned to California State University at Fullerton several years ago where I am pursing a degree in Business and accounting. My additional interests include underwater photography and skiing.

| Dr. Lum |

Dr. Lum began practicing medicine over 15 years ago, working in Los Angeles and Orange County. A Southern California native, he is proud to serve the same communities he trained in and currently resides in.
Beginning his education at UCLA, Dr. Lum graduated with a degree in Biochemistry. He then attended the Chicago Medical School/Finch University of Health Sciences where he received his medical degree. His training was completed at the Presbyterian Intercommunity Hospital. He has also been a Board Certified Family Physician ever since. Prior to his current practice, Dr. Lum has served as the Vice chairman for the Department of Family Practice at Friendly Hills Medical Group. He currently also acts as Qualified Medical Examiner (QME) for the State of California.
When not at work, Dr. Lum enjoys spending time with his wife and children well as camping and fishing.
